Code 94 is the one that runs the other way. The payer's allowed amount was higher than what you charged, so it is recording the difference rather than paying it.

- You billed less than the payer was prepared to allow. Payers do not pay more than the charge, so the extra is recorded and not paid.
- Who owes the money
- Nobody There is no money to allocate. Nothing was denied and nobody owes anything. The line is informational.
- Is appealing worth it
- There is nothing to appeal. The useful response is to look at your fee schedule, because a 94 is a payer telling you it would have paid more.
Treat this as a fee schedule signal, not a claim to work. A single 94 means nothing. The same procedure producing a 94 with several payers means your charge for it is below what the market allows, and every claim for that procedure is leaving money behind. That is worth a review, and it is the only useful thing this code tells you.
